Instruction for Ordering DVD's
- Download DVD Order Form:

Complete the order form indicating the quantity of each DVD you wish to order and the total amount of money submitted in payment.
- Send completed order form including your method of payment: check, money order, credit card (please include information on form: number, name on card, expiration, signature), or purchase order for the total amount.
Purchase orders are accepted only if the purchase amount is greater than $10.00.
